How to Enter
• Submit the form on TestStrips4Money.com and receive an order number. You must have an order number to enter.
• Or call us and we will complete the form for you so you receive an order number.
• Ship your items as usual.
Eligibility Window
• Packages must be delivered to us between November 3, 2025 and December 14, 2025.
• Minimum payment per package. $100.
• One entry per household based on physical address.
• Entries tie to order numbers only.
Terms and Conditions
• Sponsor. TestStrips4Money.com.
• Promotion period. November 3, 2025 through December 14, 2025. Eligibility depends on the delivery date to our facility.
• Entry. Only orders created through our website form or by phone with our team are eligible. Each eligible order creates one entry tied to its order number.
• Minimum to qualify. Package must lead to a payment of at least $100. Per package. Not cumulative.
• Limit. One entry per household based on physical address.
• Prizes. One PlayStation 5. Two cash prizes of $250 each. Five cash prizes of $100 each.
• Prize delivery. PS5 ships USPS with signature on December 17, 2025. Cash delivered by business check, Zelle, or PayPal based on the winner’s choice.
• Winner selection. Random draw on December 17, 2025 using eligible order numbers received during the promotion period.
• Notification and claim. We will attempt contact by email and phone. Winners have 5 days to respond. If unclaimed, the prize is forfeited. No alternate winner.
• Taxes. Winners are responsible for any taxes or reporting.
• Publicity. By accepting a prize, winners agree to the use of first name, last initial, and city for announcements.
